Apply For Funding

The form below is intended to let us learn more about the potential recipient of the financial assistance. Please answer all of the questions thoroughly to provide us with the best information possible. Applicants must live in Colorado and be under 21 years old. In addition to this form, all applications need to include the following requirements:

• A letter of justification from a TVI or Ophthalmologist stating the need and intended use for the funding.
• Grants will be awarded for up to $2,000 per child. Please note that we currently receive more applications than we are able to fund.
